Volunteer Application Form
Each volunteer must complete an application form annually regardless of the number of years served.
Select USA or CAN currency, and pay $10 Application Fee
$10 Application Fee/family [parent(s) and child(ren) 18 and under]. The Application Fee is your LAMP membership which entitles you to vote at the AGM and to receive LAMP publications.
Criminal Record Check Requirement
(If 18 years or over, background checks need to be renewed every 3 years.)
Criminal record checks must be completed, approved, and on file at the LAMP office at least two weeks prior to the start of your mission trip. LAMP requires that volunteers have a clear criminal record check in order to be accepted as a volunteer.
US CITIZENS: Criminal Record Checks are done thru Protect My Ministry and the cost is $23, payable by credit card. If your church uses Protect My Ministry email shelly@lampministry.org to discuss having her get a copy from them. Otherwise, once application is completed new applicants will automatically receive an email with the link to Protect My Ministry. For returning applicants, Shelly will let you know if you require a new one.
CANADIAN CITIZENS: Criminal Record Checks and Vulnerable Sector Checks must be done through your local RCMP or city police. Shelly will be sending you a letter stating you are a LAMP volunteer to help offset or waive the cost of the check. Should you have any questions, please contact the director of mission opportunities.
